Milpitas High School Athletics will be having its mandatory fall parent/athlete meeting on Thursday August 26, 2012 at 7:00 pm in the MHS Large Gym.

This meeting is for parents and athletes who intend to tryout for a fall sport at Milpitas High School.  Items covered will be the MHS Athletic Code of Conduct, Waiver Cards, Parent’s Code of Ethics, Transportation Fee and information about tryouts. Parents and athletes will also meet with the coach of your specific team for more information.

A parent or a representative and the athlete must attend the meeting on July 26th in order for an athlete to participate in athletic contests.  Parents may sign a waiver to have a representative take their place.  The representative may be a family member who is over the age of 18 and graduated from high school or an adult, who does not need to be a family member, who is over the age of 25.  No waivers for the athlete.

There will be a make up meeting (date and time TBA), for all parents and athletes who cannot attend the July 26th meeting.

All athletes must have a physical before they may tryout for or participate on any CIF Interscholastic sport.  Milpitas High School will be offering physicals on Thursday August 2nd from 7:00 – 8:30 pm. These physicals will take place in the Small Gym at Milpitas High School.  (Cost $25.00)  The Milpitas High School physical form must be used whether having a physical at Milpitas High School or with the athlete's own physician.  Forms are available at www.milpitasathletics.com under Links / Forms.

All athletic paperwork is due by the close of physicals (8:30 PM) on Thursday August 2nd in order to participate on August 10 or 13, 2012.  After that date there will be a 3-day waiting period, after the paper work is turned in, before an athlete may start participating.  Paperwork can be turned into the Counseling Office of Milpitas High School, the MHS Athletic Office, or to any MHS Fall Sport Coach. All athletic paperwork, including the physical form, must be obtained on line at www.milpitasathletics.com  under  Links / Forms.

Fall sports at Milpitas High School are Boys’ and Girls’ Cross Country, Varsity and Frosh/Soph Football, Girls’ Varsity and JV Tennis, and Girls’ Varsity, JV, and Freshman Volleyball.  See above for date ant times of the first try-out or practice

A $100.00 "Fair Share" Athletic Donation is requested from all players per sport. The "Fair Share" Donation Form must submitted prior to the athlete being allowed to participate in athletic contest, regardless if a donation is submitted. The donation is essential for MHS to continue to provide athletic teams for their students.

The link for the "Fair Share Donation" form does not work. Please help!

Hi Star, I'm not sure which link you're referring to because I don't see a "Fair Share Donation" link in this article. However, I did a lil' search and found the form online. The donation form can be found on page 6 (second and third form document) and page 9 (first sport document) of the two attached documents (viewable to the right of the article). Hope this helps.

Thanks Corinne, Your correct there was not a "Fair Share Donation" link. I did find the same in the documents but there is not so much as actual form (like to sign) that they are asking for, so it had me a little confused. On the paper you are referencing that talks about the "Fair Share Donation" I noticed it does say "see back", Which I don't see. :( thanks so much again for the info.
